Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community.
As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 90k people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 800 senior participants.
Schedule : Monday-Friday- 7 : 30 / 8 : 00am-4 : 00 / 4 : 30pm.
ROLE OVERVIEW and PURPOSE
The Activities Coordinator or Recreation Therapist (RT) will work together as part of a multi-disciplinary team to provide all-inclusive PACE care for the elderly. They will plan, supervise, and implement therapeutic and recreational programs, building a sense of community and a warm social atmosphere for participants. By evaluating the needs of the PACE participants, they will develop group and individual programming to meet those needs, ensuring a supportive and engaging environment. Additionally, the Activities Coordinator will focus on coordinating and planning activities specifically for the Neighborhood Healthcare (NHC) PACE program.
RESPONSIBILITIES
- Implement group and individual activities to improve or maintain the participants’ cognitive, physical, social, psychosocial, and artistic needs
- Oversee plans, and implement activities for dementia-specific population, including daily flow of activities for small groups, one-on-one programming, and sensory stimulation
- Research new ideas and participant feedback to create new programs and activities
- Organizes community outings and special events at the PACE Center, including arranging for participant transportation, entertainment, catering, and involving other staff
- Develops a monthly calendar of PACE activities
- Evaluates and promotes PACE program plans according to participant abilities and preferences within the Adult Day Health Care (ADHC) daily flow of events
- Provides leisure education to participants and caregivers for activity modifications, using adaptive equipment, activity skill development, and recommended supplies for in-home use
- Maintains general order in the activities area, including daily clean-up, routine supply inventory, ordering, and overall general budgeting
- Conducts an initial in-person comprehensive activities assessment and coordinates with the PACE Interdisciplinary Team to develop a comprehensive plan of care for each participant
- Conducts an in-person reassessment semi-annually and as needed
- Attends and participates in team and PACE IDT meetings to communicate participant changes, collaborate on participant plan of care decisions, and coordinate 24-hour care delivery
- Maintains participant medical records according to charting and reporting requirements
- Assists participants with non-skilled Activities of Daily Living (ADL), as needed
- Develops specific therapeutic programs as part of the plan of care to include recreational and social activities
- Records, signs, and dates progress reports in the participant medical records
- Develops, implements, and supervises the PACE Center Activity Program
- Assesses participants’ activities involvement and work with facility staff
- Communicates the therapeutic needs of participants with skilled nursing or assisted living facilities
- Coordinates volunteer activities at the PACE Center
- Builds within the PACE Center a sense of community and a warm social atmosphere in which participants can identify a support system
- Participates in PACE quality assurance processes
- Coordinates with various community resources and serves as resource for staff

Pay ranges :
Activities Coordinator : $24.72 - $32.14 per hour depending on experience, education and additional qualifications.
Recreational Therapist : $29.71 - $40.96 per hour depending on experience, education and additional qualifications.
Compensation Disclosure :
The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to : the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs.
As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
