Community Health Worker 1 – Latinx Outreach

Position Summary:
Promote health in the Latino/a community by building trusting relationships with neighbors and community partners in Minneapolis. Build relationships, provide education, and address barriers to access to primary health care and behavioral health services. Work collaboratively supporting people to become patients and to engage in healthy behaviors.
Supervision Received: CH Coordinator or CH Manager

FLSA Definition (Hours/Week): 40hr.; Full-Time; Non-Exempt
Duties and Responsibilities:
- Leads project(s) related to improving Latino/a health in Minneapolis and close suburbs. Manages tasks to maintain progress on a work plan. Coordinate with other staff to complete assignments.
- Delivers health education one-to-one or in group settings on a variety of topics (after receiving training) including but not limited to family-planning, unintended pregnancy prevention, sexually transmitted infections, chronic disease, mental health, and opioid harm reduction.
- Develops trusting relationships with community members and patients, creating an appropriate relationship to deliver education about sensitive topics.
- Works with patients to support and monitor care, assess resources and provide support.
- Works on one or more specific health projects with training, deadlines, and deliverables completed in a timely manner.
- Provides outreach and education at community sites and agencies, health fairs and events. distribution of flyers, etc.
- Tracks and reports data for activities and patients. Documents pertinent information in electronic medical record. Gathers and reports data needed from activities they perform.
Skills/Tools needed:
- Spanish and English fluency, written and oral.
- Basic computer literacy, ability to work with word documents, spreadsheets, patient management system, electronic health record, and patient portal, as needed to carry out essential job functions.
- Works collaboratively and respectfully with staff and others.
- High cultural competence and ability to work respectfully with individuals and groups from diverse backgrounds, including but not limited to socio-economic, race and ethnicity, nationality, and religion, both in-clinic and in the community.
- Flexibility to work evenings and weekends (up to 8 hours a week). Works at any or all NHS clinic locations, as needed.
- Ability to travel to multiple program sites or clinics throughout a day. Valid driver’s license, insurance and vehicle is very helpful.
Required Qualifications or Abilities:
- Spanish and English fluency, written and oral.
- Minimum of HS Diploma/GED.
- Demonstrated commitment to and passion for improving the health of individuals and the community.
- Speaks, understands, reads, and writes English sufficiently to carry out all essential duties.
- Experience of at least 80 hours serving community in health or social service- related activities (volunteer or paid).
- Experience or expertise working closely within the Latino/a community.
Helpful Qualifications:
- Six months or more in-clinic or in-community experience with delivery of health education/ information to diverse populations or work in a similar area.
- Health education experience in chronic disease, reproductive health and/or family planning, chemical health, or other health issue important to community.
- Existing community connections within Minneapolis and nearby suburbs.
- Minimum of six months working or volunteering in a community outreach program and/or in a customer service position.
- Knowledge of medical terminology; insurance plans; appointment scheduling; Electronic Medical Records.
- Demonstrated experience in conducting community outreach, experience, and comfort with public speaking.
- Familiarity with North and/or Northeast Minneapolis communities or Anoka County.
- Promotora de Salud, Community Health Worker Certification or Medical Assistant Certification.
Pay Range: $18-$22
