Bilingual Patient Support Navigator

Your Key Responsibilities:
- Assess patients' needs and connect them with essential medical, behavioral health, and social services, including financial assistance, housing, and shelter. Bilingual capabilities are essential.
- Provide referrals and facilitate access to local safety net services.
- Assist patients in navigating paperwork and overcome barriers to care such as transportation, insurance issues, and language differences.
- Educate patients about available services and offer guidance to help alleviate anxiety regarding their care.
What You Bring to the Role:
- An associate or bachelor's degree in a relevant field, or equivalent experience.
- 1-2 years of experience in healthcare, behavioral health, or social services.
- Experience working with low-income, unhoused, or substance abuse populations.
- Proficiency in computer applications and a willingness to learn new systems.
- Willingness to comply with OSHA and public health vaccination requirements, including TB, COVID-19, and Influenza (unless exempt).
