Case Manager
You are a compassionate and detail-oriented professional with experience in social services, mental health, and financial case management.
Key Responsibilities:
- Manage public benefits and personal finances for vulnerable populations.
- Process applications for financial assistance and Coordinated entry enrollments.
- Conduct client assessments and referrals to social services.
- Collaborate with a multidisciplinary team to expand services.
- Attend regular meetings with the Social Security Administration.
Requirements:
- Experience working with Social Security Benefits (SSI, SSDI).
- Experience as a Representative Payee.
- Background in mental health services or working with individuals experiencing homelessness.
- Strong organizational skills, empathy, and attention to detail.
- Compliance with OSHA requirements, including a negative TB test and COVID-19 vaccination (or exemption).
