The case manager is responsible for coordination of services through the continuum of care, balancing quality patient care with cost-effective utilization of resources.
Responsibilities:
- Coordinate client-centered care that is safe, effective, efficient, and equitable.
- Develop effective working relationships with the medical team throughout the case management process.
- Handle case assignments, review case progress and determine case closure.
- Assist patients in making informed decisions by acting as their advocate regarding their treatment options.
- Communicate with the patient’s health plans through the pre-certification process to secure reimbursement for the hospital services rendered.
- Performs clinical reviews to insurance providers to facilitate reimbursement for the delivery of the patient care services provided.
- Minimum RN, LPN, or Social Worker licensure
- 2 years case management experience preferred.
- Excellent communication, organizational, and time management skills.
- Excellent knowledge of case management principles, healthcare management and reimbursement.
