Job Summary
The LPN specializing in Geriatrics provides nursing care and support to elderly patients in various healthcare settings. They assist in the assessment, planning, implementation, and evaluation of nursing care for geriatric patients under the supervision of a registered nurse or physician.
Key Responsibilities
- Provide direct patient care to geriatric patients, including assistance with activities of daily living (ADLs), medication administration, wound care, and monitoring vital signs.
- Perform comprehensive health assessments of geriatric patients, including physical, cognitive, and psychosocial assessments.
- Assist in developing individualized care plans for geriatric patients based on assessment findings and in collaboration with the interdisciplinary healthcare team.
- Administer medications as prescribed by healthcare providers, ensuring accuracy and adherence to medication protocols.
- Monitor patients' conditions, report any changes or concerns to the supervising nurse or physician, and participate in the evaluation of patient outcomes.
- Educate patients and their families about health conditions, medications, treatments, and preventive care strategies relevant to geriatric health.
- Maintain accurate and up-to-date patient records, including nursing assessments, care plans, progress notes, and medication administration records.
- Collaborate with other members of the healthcare team, including registered nurses, physicians, therapists, social workers, and other support staff, to ensure comprehensive and coordinated care for geriatric patients.
- Adhere to legal and ethical standards of nursing practice, as well as facility policies and procedures, ensuring patient safety, privacy, and confidentiality at all times.
- Stay informed about current trends, best practices, and evidence-based interventions in geriatric nursing through ongoing education and professional development activities.
