Integrated Case Management Supervisor – RN or Social Work Case Manager
Job Summary:
Networks Connect is seeking a dynamic and experienced Integrated Case Management Supervisor to lead and oversee daily operations within our Integrated Case Management department for our Client. This role requires strong leadership skills to foster collaboration among Case Management staff, Physician Advisors, and healthcare providers, including Physicians, Advanced Practice Professionals, and Medical Residents. As a key leader, you will set the standard for excellence, ensuring the successful execution of team projects, responsibilities, and goals that align with our organization's mission.
Key Responsibilities:
- Supervise and support the Integrated Case Management team in achieving department objectives.
- Establish effective working relationships with medical staff and interdisciplinary teams.
- Ensure compliance with healthcare regulations and case management best practices.
- Promote a culture of collaboration, continuous improvement, and professional development.
- Lead case management initiatives to improve patient outcomes and optimize care coordination.
Requirements:
For Registered Nurse Case Manager (RNCM):
- Education: Master's of Science in Nursing (MSN) or Bachelor of Science in Nursing (BSN) from an accredited institution.
- Licensure: Active State of Florida Registered Nurse license.
For Social Work Case Manager (SWCM):
- Education: Master of Science in Social Work (MSW) from an accredited institution.
Additional Requirements:
- Certification: Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification required.
- Experience: A minimum of five (5) years of healthcare and case management experience in a hospital or clinical setting.
Preferred Qualifications:
- Strong leadership skills with the ability to motivate and manage teams effectively.
- Proven ability to analyze problems, develop solutions, and follow through with implementation.
- Independent decision-making, critical thinking, and problem-solving skills.
- Excellent verbal and written communication skills across all levels of an organization.
- Experience in cost center-based management, group processes, and teamwork.
