Network ESC -
Private Medical Facility is in need of a Bilingual Patient Navigator.
Monday through Friday
8.30am-5pm
Patient navigators work one-on-one with patients to encourage them to adhere to medical treatment and access social services. Also workwith a variety of other people, including insurance companies, employers and case managers. Help patients navigate the healthcare system, from screening to treatment and follow-up. They assist patients and their families in many ways, including
- Communication : Helping patients communicate with healthcare providers and other service providers
- Suppor t : Helping patients get financial, legal, and social support
- Care coordination : Helping patients identify and prioritize their needs, and addressing service gaps
- Resource materials : Collecting and distributing resource materials to help patients understand their care needs
- Enrollment : Helping patients enroll in research or other healthcare initiatives
- Problem resolution : Investigating, documenting, and resolving problems and conflicts
- Process evaluation : Evaluating processes and making suggestions for improvement
