The On-Call Transplant Nurse provides vital clinical support during after-hours operations, ensuring timely and effective management of organ offers, patient triage, and urgent clinical issues. This role is integral to maintaining continuity of care, supporting transplant surgeons and nurses, and delivering high-quality patient care outside of regular business hours.
Key Responsibilities:
- Clinical Nursing Support for Organ Offers
- Receive and manage organ offer calls after hours, ensuring accurate documentation and assessment.
- Communicate promptly with the on-call transplant surgeon or nurse to facilitate decision-making.
- Accurately document organ offers and all associated information in Renown EPIC as an affiliate user.
- Patient Triage and Urgent Clinical Issue Management
- Respond to patient calls, triaging based on severity and urgency.
- Address critical lab values and urgent medical issues, ensuring immediate intervention when required.
- Communicate and collaborate with the on-call transplant nurse or surgeon to ensure appropriate follow-up and care.
- Document all patient interactions and updates in EPIC for seamless handover and continuity of care.
- Non-Urgent Call Management
- Utilize a structured after-hours phone tree to route non-urgent calls appropriately.
- Direct pharmacy refill requests and non-urgent inquiries to regular business hours.
- Provide clear instructions to callers regarding urgent and non-urgent matters.
Qualifications:
Education: Registered Nurse (RN). BSN preferred.
Experience: Minimum of 2 years of experience in transplant coordination, critical care, or acute care nursing.
Familiarity with EPIC or similar EMR systems is highly desirable.
Skills and Abilities: Strong triage and clinical decision-making skills under pressure.
Exceptional communication and collaboration skills to work effectively with multidisciplinary teams.
Ability to multitask and prioritize in a fast-paced, on-call environment.
Work Schedule: This is an on-call position requiring availability during nights, weekends, and holidays. Hours 5pm-8am PST.
