Overview: We are seeking a dedicated and compassionate Case Manager to join our team, specializing in working with adults experiencing serious mental illness. The ideal candidate will have a strong background in mental health services, possess excellent interpersonal skills, and be committed to promoting the well-being and independence of individuals with severe mental health challenges. New Beginnings Bluegrass is a non-profit agency in Lexington, Kentucky that is dedicated to improving the lives of individuals with serious mental illness by providing quality housing, and community-based support services.

Responsibilities:

Case Management, Assessment, and Service Planning:

  • Provide face-to-face services in a location of the client’s choosing to provide linkages to services, promote community integration, develop meaningful daily activity, and assist with implementing and achieving the client’s personal recovery goals.
  • Conduct ongoing assessments to determine client needs, strengths, and barriers to independent living.
  • Collaborate with clients and their support systems to develop person-centered plans.
  • Identify and prioritize goals, outlining measurable and achievable objectives to support recovery.
  • Evaluate clients’ progress in attending and meeting Activities of Daily Living (ADLs).

Advocacy, Collaboration, and Resource Coordination:

  • Advocate for clients within the community, ensuring access to appropriate physical and mental health services and resources.
  • Coordinate and link clients to community resources, social services, housing, employment, and other essential supports.
  • Collaborate with mental health professionals, recovery teams, and community agencies to facilitate a cohesive and holistic approach to care.

Client Empowerment and Skill Building:

  • Empower clients to develop and enhance life skills, coping mechanisms, and resilience.
  • Provide psychoeducation relating to health management, individual strengths, and community resources.

Monitoring and Crisis Intervention:

  • Regularly monitor clients' progress towards established goals and intervene promptly in times of crisis.
  • Work closely with crisis intervention teams, mental health professionals, and emergency services when needed.

Documentation:

  • Maintain accurate and up-to-date documentation, ensuring confidentiality and compliance with regulatory standards.
  • Utilize electronic health record and other documentation systems.

Qualifications:

  • Bachelor's degree in psychology, social work, or other behavioral health related field.
  • Knowledge of mental health disorders, treatment modalities, and community resources.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative team.
  • Familiarity with relevant state and federal regulations.
  • Must have a valid driver’s license, proof of current auto insurance, and reliable transportation.

Preferred:

  • At least one year of full-time employment experience working directly with adults in a human service setting post degree. A master’s degree in psychology, social work or other behavioral health related field may be substituted for this experience.
  • Experience working with adults with serious mental illness (i.e., schizophrenia).
  • Certification as a Targeted Case Manager.
