Day Program Facilitator

```Job Overview:```
We are seeking compassionate and dedicated individuals to join our team as Day Habilitation Providers. You will provide access to community engagement while supporting individuals with developmental disabilities or autism. This is a rewarding opportunity to make a positive impact on the lives of others.

```Duties:```
- Assist individuals during community activities.

- Assist with creating individualized activity plans that promote skill-building and personal growth.

- Facilitate individual activities that encourage socialization engagement, and communication.

- Monitor and document progress towards goals and report any changes to the supervisory staff.

- Provide transportation to and from the day program as needed. - Maintain a safe and clean environment for participants.

- Collaborate with families, guardians, and other professionals to ensure comprehensive support for individuals in care.
- Follow all applicable policies, procedures, and regulations, including HIPAA guidelines

```Qualifications:```
- Experience working with individuals with developmental disabilities or autism is preferred
- Knowledge of behavior management techniques and strategies
- Familiarity or ability to become familiar with community activities for disabled population.
- Written and verbal communication skills to effectively interact with individuals, their support team, external staff, and write notes.
- Flexibility to adapt to changing needs and schedules
- Valid driver's license and reliable transportation
