Essential Responsibilities and Target Outcomes:

PSH Case Management (2) and 1 Lead Responsibilities:
- Manage a caseload of up to 50 PSH Residents
- Creation and implementation of Personal Housing Plans for caseload clients, including
- Complete Document readiness process for new tenants and tenants annual
- recertification
- Complete and maintain lease agreements with project tenants
- Provide linkage to community resources
- Inform and build upon treatment plans that increase housing retention among the residents through regular case consultations.
- Help promote policies and procedures as they relate to services that improve resident Activities of Daily Living (ADLs).
- Collaborate with the Operations Department to ensure the safety and maintenance
- of the facilities and provide leadership during facility emergencies.
- Manage conflicts and personal emergencies experienced by the residents.
- Provide direct service to the residents including life-skills coaching, transportation, and inter-personal conflict resolution.
- Develop and maintain relationships with residents, staff and local service providers and participate in community meetings.
- Completes all necessary intake paperwork (internal and HMIS), collects all clients identifying documentation and provides an initial program orientation
- Develops an individual case management plan, including financial planning and housing stabilization goals collaboratively with each client
- Completes Occupancy Fee form for clients who generate an income, as applicable
- Inputs client data in the Homeless Management Information System (HMIS) and other data management systems accurately and in a timely manner, within 24 hours of a service provided
- Works with client to track progress toward goals in detailed case files with charts, progress notes, and outcome evaluation. A minimum once a month meeting with the clients is expected
- Makes appropriate community referrals and provides support to connect to ancillary services
- Helps client with all housing related matters including assistance with understanding their lease agreement.
- Assist clients with SSI/SSDI, medical, counseling, employment and other related needs or concerns by connecting them to appropriate community resources and referrals. Arranges supplemental transportation assistance as necessary.
- Ensures client compliance of program policies and expectations and provides warnings in verbal and written form, as necessary
- Keeps detailed case files including information about clients, program expectations and client progress notes and documentation
- Conducts unit inspections once a month for cleanliness, safety and compliance
- Completes monthly check requests for rent and other billable expenses and internally tracks all expenses requested/billed in Appfolio
- Develops and maintains a list of potential housing opportunities for clients with support from other staff
- Collaborates and communicates with other service providers, community partners and other agency staff/property managers as it relates to individual work or client needs
- Completes internal reports as assigned in a timely manner as required
- Completes specific grant-related reports/documentation as required
- Performs other duties as assigned

Minimum Requirements:
Education:
· Associate's Degree in Psychology or two (2) years of relevant case management experience or experience within the Miami Dade Homeless Trust CoC
· Familiarity with Microsoft Office, ability to efficiently learn and navigate computer
· systems
· Certified Recovery Peer Specialist preferred
· Bilingual English/ Spanish preferred
