What are the responsibilities and job description for the Residential Substance Abuse Case Manager position at New Hope Corps?
· Assist with the coordination and documentation of client linkage to care.
· Act as a liaison between health care providers, courts, families, and outside agencies.
· Provide case management services in accordance with the service plan as appropriate to the program.
· Coordinate with the members of the Treatment Team and provide them the appropriate reports prior to the meeting.
· Completes documentation and reports as necessary.
· Attends staff meetings weekly and participates as a team member.
· Upholds total confidentiality with respect to clients in all instances.
· Attends training and in-services as required.
· Performs other duties as appropriate and assigned.
· In case of emergency, hurricane, or other disasters, the employee is willing, if needed, to evacuate with the clients to the appropriate facility.
· Become familiar with the billing process for Medicaid.
Salary : $42,000
