What We Do
The Care Coordination Program provides services as designed by the NYS DOH Health Home Model, based on unified systems of care to coordinate and integrate physical and behavioral health care, substance use treatment, and social services. The goal of Care Coordination is to minimize the use of hospitals and emergency rooms by providing access to all of the services an individual needs to assure he or she stays healthy.
Job Summary
The Intake Specialist is responsible for the initial outreach and enrollment of clients for the agency through provider relationships and individual engagement. The successful Intake specialist will achieve a high rate of successful enrollments. You will also :
- Conduct aggressive outreach in the field and via phone to prospective Health Home clients as well as complete follow up, as needed.
- Conduct outreach and provide clients and internal or external providers with an overview of New Horizons Counseling Center Health Home model.
- Complete client detail intake pre-screenings, Medicaid eligibility, explanation of Health Home
- Conduct intakes with client's in hospital, shelter, office, and home settings
- Conduct case conferences with referral sources to identify client needs
- Enter progress notes for outreach and enrollments
- Achieve a conversion rate comparable or above industry standard
- Participate in staff meetings and trainings

- Perform other duties as requested.
