Position Overview
The Career Services Case Manager is responsible for providing comprehensive support to individuals seeking employment or career development opportunities. This includes conducting assessments, hosting hiring events, attending job fairs, creating personalized employment plans, building employment/community partnerships, connecting clients to job resources, providing ongoing coaching, provide career workshops, and ensuring long-term job retention. The Case Manager will collaborate with clients, employers and community resources to overcome barriers to employment, track progress, and maintain accurate documentation throughout the process.
Key Responsibilities
· Client Assessment & Intake: Conduct thorough assessments to identify clients’ needs, strengths, career interests, and barriers to employment.
· Individualized Employment Plans: Develop and implement tailored employment plans with clients based on their skills, experience, and career goals.
· Job Search Support: Assist clients in job search activities, including resume writing, cover letter preparation, interview coaching, and job application submission.
· Job Placement: Work with local employers to identify job openings and match clients with appropriate employment opportunities.
· Plan and produce three hiring events annually to connect job seekers with employers on a large scale.
· Job Retention Support: Provide post-placement support, including check-ins and troubleshooting any issues that may arise to ensure job retention and client success.
· Case Management: Maintain ongoing communication with clients to monitor progress, provide guidance, and address any challenges that may arise during the employment process.
· Referral & Resource Coordination: Refer clients to appropriate community resources, training programs, or educational opportunities to enhance employability.
· Documentation & Reporting: Maintain accurate, timely, and confidential records of client interactions, services provided, and outcomes. Prepare reports for internal and external stakeholders as required.
· Collaboration: Work closely with colleagues, other case managers, social workers, and community partners to ensure clients have access to the resources and support they need.
· Workshops & Group Services: Lead or facilitate group sessions on job readiness, career development, and related topics.
· Advocacy & Support: Advocate for clients' needs in securing accommodations, addressing challenges, and overcoming employment barriers.
Qualifications
· Strategic leader who is goal oriented, self-starter, energetic, creative, and a team player.
· Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (preferred, not required).
· Minimum of 1 years of experience in case management, employment services, or a related field.
· Strong knowledge of labor market trends, job search strategies, and employer expectations.
· Strong communication, interpersonal, and problem-solving abilities, with a proven capacity to present professionally to stakeholders at all levels.
· Ability to work with diverse populations, including individuals facing barriers to employment such as homelessness, disabilities, or criminal records.
· Experience in developing and implementing individual service plans.
· Knowledge of community resources and support systems.
· Proficiency in Microsoft Office Suite and case management software, including client Track and Sales Force. (Preferred)
· Strong organizational skills and attention to detail.
Working Conditions
· Full-time position, office setting.
· Occasional evening or weekend hours may be required for client meetings or events.
· Some travel may be required for outreach, client meetings, or employer partnerships.
Job Type: Full-time
Pay: $45,000.00 - $55,000.00 per year
