The Maternal Health Program's main purpose is to assure that all pregnant women have the best possible health care, and that New Mexico's babies have the best possible start in life. This posting may be used to fill positions in multiple locations.
Why Does the Job Exist?
The CARA Navigator role ensures compliance with the CARA state law, effective July 1, 2019, which mandates the creation of a Plan of Care for every newborn with in-utero substance exposure. This position supports families by offering services to parents with substance use challenges and their infants.
How Is the Job Accomplished?
Plan Review and Coordination:
- Review and ensure the completeness of every Plan of Care.
- Capture and document insurance details for mothers and newborns to facilitate accurate care coordination referrals.
Family Assessments and Home Visits:
- Conduct comprehensive family assessments to identify needs and update Plans of Care accordingly.
- Perform home visits to engage families and support their access to services.
Collaboration and Compliance:
- Coordinate with all parties involved in the Plan of Care to ensure compliance with state laws and CARA regulations.
- Notify Statewide Central Intake (SCI) when safety concerns arise.
Support Services and Outreach:
- Provide transportation for families to access identified support services.
- Deliver training and technical assistance to providers on CARA and Plans of Care.
- Conduct outreach to target populations statewide.
CARA Workgroup Management:
- Lead the ongoing CARA workgroup by developing agendas and facilitating discussions.
- Collaborate with state agencies to implement CARA across various sectors.

Ideal Candidate
Knowledge in the following areas:
- Potential impact of substance uses and substance exposure on individual and family functioning.
- Possess a working knowledge of resources in the community, as well as how to facilitate connection to resources, that have the capacity to support the family in maintaining stability.
- Experience working with individuals who have experienced substance abuse.
- Have knowledge of, and experience working with individuals from a strength-based de stigmatization approach.
Minimum Qualification
Bachelor's degree in social work, Psychology, Guidance and Counseling, Education,
Sociology, Criminal Justice, Criminology, and/or Family Studies/Services and four (4) years of any combination of experience including working with communities, working on health or social service-related matters, social work/case management experience, behavioral health, and/or health care. Any combination of education from an accredited college or university in a related
field and/or direct experience in this occupation totaling eight (8) years may substitute for the required education and experience.

Must possess and maintain a valid Driver's License.
Working Conditions
Work is performed in an office setting: late hours and weekends work may be required. Will be exposed to regular periods of video display terminal and keyboard usage and stressful situations. Travel may be required. Incumbent will work under stress and frequent time constraints. Must have own mode of transportation. Incumbent may be required to furnish a personal vehicle for carrying out assignments.
