ICM Program Manager

Full Time Position

Monday - Friday Schedule, Standard Business Hours

 

Purpose: 

This position is responsible for the program management of the agency’s Intensive Case Manager Team: monitoring and execution of the program management, adherence to contract and funder requirements, supervision of staff, and assurance of quality service and collaborative relationships with all stakeholders, including State, County, and community agencies.  Works with the agency management team to ensure implementation of best practices, positive program outcomes, participant centered care, continued quality service, upholding the New Narrative Mission and Values, Integrated and Team Based Care. This requires travel to provide oversight and supervisions for the team, as well as occasionally providing direct care to participants as needed to supplement the team. 

This program will also provide oversite of the Clifford Program as they adopt a modified ICM model. This includes supervision of the contract and coordination with Clifford Partners including IHI, DCJ, Property Management and the JOHS. This position will provide support for screenings into the building and coordinate with JOHS as the representative for the CoC contract.  

Accountabilities: (Responsibilities of the job):

  • Provides bi-weekly program supervision to supervisees; conducts group supervision in program team meetings and provides employee training as needed. 
  • Routinely audits and provides feedback on charts of individuals engaged in services and program documentation.   
  • Routinely reviews data and information tracking as designated by contract. 
  • Initiates recruiting, hiring, training, and corrective action of employees. 
  • Manages referrals, checks accuracy of enrollments in the database and oversees the program and eligibility requirements. Ensure all documentation is completed within 24-48 hours in electronic health record. 
  • Participates in committee meetings addressing quality of care. Committee review includes utilization in the following areas: services, medication management, IRs, prescriber and hospitalizations. 
  • Serves as liaison for Washington and Multnomah County staff, coordinating site reviews, audits, chart reviews, and certificate requirements.  Attends all County level meetings as required. 
  • Coordinates all funded services with County and State. 
  • Works with County staff and internal auditors to audit charts, identify required changes, and provide staff training/supervision as needed to ensure that deficiencies are corrected. 
  • Ensure CoC renewal documentation is completed and delegate and monitor HMIS documents.  
  • Collaborates to integrate Peer Delivered Services in agency services and supports. 
  • Works with Quality Assurance Manager to conduct case review and OAR compliance.  Provides feedback to staff and addresses issues of quality care and/or utilization of services through an organized performance improvement process.  Partners with other staff members to address and resolve stakeholder issues and complaints.  
  • Is in an on-call after hours rotation for after-hours consultation and crisis response.    
  • Supports and trains designated staff on processes for individual benefits and programs such as assistance for medications, housing access and resources, non-facility based respite services, substance use treatment, etc. 
  • Demonstrates strong leadership skills and support of New Narrative Mission and Values. 
  • Conducts timely employee performance reviews, provides on-going performance improvement feedback and coaching. Works closely with direct reports and management team to identify development training plans and programs.  Collaborates with the Intensive Services Director to ensure the program training recommendations align with the agency’s mission, values, culture, processes and programs. 
  • Performs other incidental and related duties as required and completes in a timely manner.

Education/Certificates: 

  • Understanding of SPMI and other mental health diagnoses, systems of care, trauma informed care, and DEI principles and inclusivity.  
  • Master’s Degree in Psychology or related field, credential through MHACBO and licensure through respective Board. CADC preferred. 

Licenses/Registration: Applicant must be a Qualified Mental Health Professional with a Master’s Degree in Psychology or related field.  A minimum of five years post-graduate as well as supervisory experience preferred.  Must have a valid CPR and First Aid Certificate. Licensed with respective Board (LPC, LCSW) preferred. 

Experience/Qualifications: Applicant must possess the skills necessary to work independently. Must have a clear understanding of medications and the appropriate applications.  Knowledgeable on all administrative rules and other applicable laws and regulations, along with the acceptable program practices and guidelines.
