The CDO Support Broker/Case Manager is responsible for coordinating services for individuals that have selected Consumer Directed Options, a choice under the Supports for Community Living, Michelle P. and Acquired Brain Injury waivers. The CDO Support Broker is a positive team player that promotes a culture of inclusion, and participation.
Required Education and Experience
- Bachelor’s degree in a human services field.
- Valid driver’s license, reliable operating transportation, and proof of current auto insurance
Preferred Education and Experience
- Experience working with IDD population and Medicaid Waiver programs
- SCL (Supports for Community Living) Intellectual Disability Professional
Essential Functions

Case Management
- Complete assessments to determine eligibility for ongoing services
- Maintain client plan of care that is reflective of needs as identified by interdisciplinary team
- Develop crisis prevention plan and connect participants with additional community-based services to help prevent institutionalization
- Process employee paperwork and coordinate training prior to service provision
- Complete required documentation and training
- Participate in and promote performance improvement activities
Cost Containment and Productivity
- Work collaboratively with other region-wide programs to determine services offered in the Region
- Ensure that staff and other resources within the Region are utilized in a manner that optimizes the service delivery system
Corporate Involvement and Responsibilities
- Actively participate in corporate-wide initiatives
- Support and work within the mission and strategic plan of the organization
- Ensure compliance with relevant laws, regulations and standards. e.g., state contracts, Joint Commission, etc.
