We are seeking a dedicated and compassionate Success Coach (Case Manager) to support individuals and families in achieving their goals. Using the Whole Family Approach and Family-Centered Coaching Principles, you will guide clients in securing housing, employment, and essential resources to improve their lives.
What You’ll Do
- Provide direct coaching to clients and families, meeting them where they are.
- Offer education, guidance, and referrals tailored to each client’s unique needs.
- Collaborate with internal teams and funders to maximize positive client outcomes.
What You Need
Required
- Strong communication and customer service skills.
- Experience providing resources to families.
- Ability to make independent decisions based on funder and agency requirements.
- Valid driver’s license
Preferred
- Associates Degree in Human Services or related field.
- Experience with workshop development.
- Background in community outreach and event coordination.
