WHAT YOU WILL BE DOING (ESSENTIAL DUTIES) …

We are seeking an experienced Case Manager to join our Emergency Services Team as a Mobile Response and Stabilization Services (MRSS) Case Manager. The MRSS Case Manager is responsible for providing immediate, on-site crisis intervention and stabilization services to individuals experiencing behavioral health emergencies. This role involves rapid response to crisis situations, comprehensive assessment, and the development of stabilization plans to ensure the safety and well-being of clients. The MRSS Case Manager works collaboratively with a multidisciplinary team to deliver integrated care and support to clients and their families.

Main Duties and Responsibilities :

  • Respond promptly to crisis calls and provide on-site assessment, de-escalation, and stabilization services.
  • Conduct thorough assessments of clients' behavioral health needs, risk factors, and strengths.
  • Develop and implement individualized stabilization plans in collaboration with clients, families, and other service providers.
  • Coordinate with healthcare providers, social services, and community resources to ensure comprehensive care and support for clients.
  • Provide follow-up support and case management to ensure continuity of care and monitor clients' progress.
  • Maintain accurate and timely documentation of all client interactions, assessments, and interventions.
  • Advocate for clients' needs and rights within the healthcare system and community.
  • Educate clients, families, and the community about behavioral health issues and available resources.
  • Participate in multidisciplinary team meetings and case conferences to ensure integrated and coordinated care.
  • Requirements :

    WHAT WE EXPECT OF YOU…

  • Bachelor's degree in social work, psychology, counseling, or a related field.
  • Minimum of 2 years of experience in crisis intervention, case management, or behavioral health services.
  • Valid Driver’s License and reliable transportation to travel throughout the area.
  • Strong assessment and crisis intervention skills.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in high-stress situations.
  • Knowledge of community resources and social services.
  • Proficiency in electronic health records (EHR) and documentation.
