The Social Determinants of Health (SDOH) Coordinator at Newport Mental Health (NMH) will lead efforts to foster an inclusive, equitable, and culturally competent environment within our Certified Community Behavioral Health Clinic (CCBHC). This position is integral to aligning SDOH efforts with CCBHC standards, developing and implementing training programs, and establishing partnerships to support both internal staff development and broader community engagement. The SDOH Coordinator will also play a key role in building partnerships and coordinating care agreements to enhance service access, particularly within underserved populations.

WHAT YOU WILL BE DOING (ESSENTIAL DUTIES) …

  • Design, deliver, and assess training programs on SDOH principles, cultural competency, and CCBHC standards to ensure all staff are equipped to provide high-quality, inclusive care.
  • Partner with external SDOH trainers and consultants as needed to deliver comprehensive training programs.
  • Assist in the creation and implementation of a SDOH strategic plan that supports CCBHC initiatives and complies with federal Culturally and Linguistically Appropriate Service standards (CLAS).
  • Provide resources and materials that support SDOH initiatives for CCBHC, compliance, licensure, and program requirements.
  • Establish partnerships with local community organizations and equity-focused groups to improve access to services, particularly for BIPOC communities and other underserved populations.
  • Develop and manage care coordination agreements that facilitate cross-organization referrals, ensure continuity of care, and enhance resource sharing.
  • Lead monthly SDOH committee meetings, working collaboratively with staff to identify priorities, monitor progress on SDOH goals, and develop actionable initiatives that promote an inclusive workplace.
  • Prepare SDOH-related documentation required for compliance with CCBHC and other regulatory bodies.
  • Represent the organization at community events and forums to raise awareness of our SDOH and CCBHC initiatives.
  • Work with the communications team to promote community initiatives through newsletters, social media, and internal channels.
  • Requirements :

    WHAT WE EXPECT OF YOU…

  • High School Diploma or GED, bachelor’s degree preferred in social sciences, human resources, public health, or a related field
  • Minimum of five years of professional experience, particularly within healthcare, behavioral health, or community service settings.
  • Experience developing and implementing SDOH programs and conducting cross-cultural training.
  • Familiarity with CCBHC standards is a plus.
  • Strong understanding of SDOH principles and demonstrated ability to apply them in a healthcare or community-based setting.
  • Experience developing, delivering, and evaluating SDOH and cultural competency training programs.
  • Proven ability to build relationships with diverse communities and external partners.
  • Strong analytical skills with the ability to translate data into strategic actions.
  • Excellent communication, facilitation, and interpersonal skills for interacting with internal teams and community stakeholders.
  • Self-initiating with good judgment and minimal need for supervision.
