What are the responsibilities and job description for the Outpatient Case Manager position at Next Life Recovery Center Ohio?
This position is responsible for managing the intake process, assisting clients with government programs, housing assistance, and facilitating communication with courts and probation/parole officers. The Case Manager ensures that clients receive the necessary support services to aid in their recovery and reintegration. Collaboration with internal and external resources is essential for successful client outcomes.
TASKS AND RESPONSIBILITIES:
1. Client Intake and Assessment:
- Conduct initial intake assessments to determine client needs.
- Develop and maintain comprehensive client records.
- Assist clients in completing necessary documentation for services.
2. Government and Housing Assistance:
- Guide clients through applications for government assistance programs (e.g., Medicaid, food stamps, disability benefits, etc.).
- Provide referrals and support in securing stable housing.
- Collaborate with housing agencies to facilitate placement.
3. Legal System Coordination:
- Act as a liaison between clients and legal entities, including courts and probation/parole officers.
- Assist clients in understanding and complying with court requirements.
- Provide necessary documentation and progress reports to legal authorities as required.
4. Internal and External Communication/Collaboration:
- Maintain effective communication with treatment teams and external agencies.
- Advocate for clients and facilitate access to essential services.
- Work collaboratively with community organizations to enhance support networks.
5. Administrative Duties:
- Ensure accurate and timely documentation of client interactions.
- Maintain client confidentiality and comply with all applicable laws and regulations.
- Participate in staff meetings and training sessions as required.
6. Professional Responsibility:
- Adhere to all applicable federal and state laws, professional codes of ethics, and agency policies.
- Maintain professional boundaries and uphold client rights.
- Pursue ongoing professional development to enhance skills and service quality.
EDUCATION:
Bachelor’s Degree in Social Work, Psychology, or a related field preferred but not required. At least High School Graduate.
EXPERIENCE:
- Experience in case management, social services, or a related field.
- Experience working with individuals involved in the legal system preferred.
- Knowledge of government assistance programs and housing resources.
- Experience working with KIPU EMR System is preferred.
QUALIFICATIONS:
- Strong organizational and administrative skills.
- Effective communication skills (both written and oral).
- Ability to work independently and under pressure.
- Knowledge of confidentiality laws related to client services.
- Familiarity with health and safety issues in social service settings.
Job Type: Full-time
Pay: $25.00 per hour
Expected hours: 40 per week
