What are the responsibilities and job description for the Case Manager position at Team Behavioral Health?
Job description
Will serve as an advocate for persons and coordinate programs of support throughout the community for the benefit of persons who suffer from mental illness. Will provide crisis intervention services as needed to reduce symptoms. Will provide case management services to link to appropriate community resources and services to prevent incarceration and hospitalization. Will work in cooperation with family members and community organizations including, but not limited to law enforcement, schools, hospital staff and other community providers.
- promoting and engaging in therapeutic activities with clients
- facilitating educational, behavioral health, or skill-building groups
- coaching clients in practical living skills such as nutrition, budgeting, and other independent living activities
- accompanying clients to appointments, community events, and outings
- Work with clients to reduce behavioral health symptoms
- Monitor school and home environment when needed.
- completing required documentation that captures the activities, goals, and progress of each client
- Hours are flexible around clients schedule. Most shifts are 1pm-9pm
- Pay is $15-17 per hour
Qualifications:
- High School diploma required (will be verified)
- Associate degree in social services or related field preferred
- One year of related mental health, social service or working with youth experience
- Drivers License, Insurance and car required
- Must pass background check
- Self-determined schedule
- Weekend availability

- Social work: 1 year (Preferred)
