Overview
We are seeking a dedicated and experienced Manager of Care Management (RN or MSW) to join our healthcare team in Fullerton, California. In this full-time position, you will be responsible for leading and managing a dynamic team of Case Managers and Care Management Coordinators. This role will focus on ensuring that patients receive exceptional care across the continuum and that resources are used efficiently and effectively.
Responsibilities
- Oversee the daily operations and clinical practice of the Care Management department.
- Manage and support a team of Case Managers and Care Management Coordinators to ensure optimal patient care.
- Advocate for patients to ensure timely access to appropriate care and support.
- Work closely with interdisciplinary teams to ensure coordination and continuity of care across various settings.
- Monitor and improve care management practices to align with healthcare goals and standards.
- Assist in the development of policies and procedures for effective patient management
Requirements
- Required Education: Bachelor’s degree in Nursing or Master’s degree in Social Work.
- Required Experience: Minimum of 3 years working in a hospital or ambulatory care setting as an RN or Social Worker.
- Required Skills: Strong project management, operations, and leadership experience.
- Preferred Education: Master’s degree in Nursing or Health Administration.
- Preferred Certifications: Case Management Certification (CCM or ACM) upon hire.
- Preferred Experience: 3 years of leadership experience in a healthcare setting.
Job Type: Full-time
Pay: $161,154.00 - $178,000.00 per year
