What are the responsibilities and job description for the Care Coordinator position at Niagara Falls Memorial Medical Center?
Niagara Falls Memorial Medical Center is looking to hire a Care Coordinator to serve at-risk and high-need primary care patients.
This position is well-suited for an individual who likes working in the health care setting and who has the ability to assess patient needs, develop and follow through on patient care plans and document patient encounters in the electronic medical record. The Care Coordinator will work collaboratively with the entire primary care team as well as with community-based organizations and service agencies.
The Care Coordinator will be responsible for managing care plans developed by other entities for primary care patients, playing an active role in the NFMMC Primary Care Network’s Social Determinant of Health program, and assisting in retaining the network’s Primary Care Medical Home accreditation.
The candidate must posses a Bachelors' degree in Social Work.
