Niagara Hospice is looking for a compassionate, certified Home Health Aide (HHA) to join our Home Care team. The HHA will provide in home HHA services to patients throughout Niagara County under Niagara Hospice care.
Schedule :
Full Time - 5 days per week (37.5 hours) 8 am - 4 pm
- Every other weekend required
Location : On the road within Niagara County
Duties & Responsibilities
Reviews patient Nurse’s Aide Care Plan and reports change in patient’s condition to RN / LPN and / or SupervisorAssist patient’s with showering and personal care assistance (including oral, hair and nail care)Assists with transfers to toilet and records output if necessaryEmpties drainage collection devices (foley catheters, leg bags, etc.)Application / removal / disposal of incontinence briefs and related productsPerforms peri-care in a manner consistent with standards of infection control and hygiene.Prepares patient for meals, assists in setting up and feeding patients, serves fresh water and nourishments when indicated.Lift, transfer, and transport patients in wheelchair, bed or gurney as directed using proper techniqueAssists with patient positioning and repositioning (turn and positioning program)Obtain and record patient’s temperature, pulse, respiration, blood pressure, urine testing, weight, intake / output as directed by the charge nurse and / or care plan.Maintains confidentiality of patient information and data when reporting to the nurse.Responsible for routine bed making and linen changesApplication of support devices, adaptive equipment, & oxygen.Post mortem care as directedDemonstrates sensitivity to patients to ensure their comfort, privacy and well-being.Participates in laundering personal patient clothing as directedOther duties as assigned by the nurse.Qualifications
High school education or equivalentA sense of personal responsibility and integrityEmotional maturity & positive attitudeGood communication, phone etiquette and interpersonal skillsRespect for the quality of lifeLicensure / Certification
