What are the responsibilities and job description for the Community Health Worker position at Nicollet County?
This position will be a frontline public health worker with a close understanding of the community they serve. The Community Health Worker will work across the Health and Human Services Department to improve quality outcomes for priority populations, including connecting community members to essential county resources and services.
Additionally, the Community Health Worker will carry a caseload of pregnant and parenting families from the prenatal stage up to age five, offering face-to-face services in home, office, and community settings. They will help strengthen parent-child relationships, promote healthy growth and development, improve family life by reducing risks, and guide parents in creating learning experiences that support their children's development.

- An Associate's degree from an accredited college or university in a related field with up to 1 year of related healthcare experience; or,
- An equivalent combination of Community Health Worker Certification and 2 years of related healthcare experience; and
- Must be bilingual in Somali.
