Overview:
We are seeking compassionate and skilled nurses to join our Nightingale Care team, providing high-quality medical care and support to one patient at a time in the comfort of their homes. As a home health nurse, you will assess patient needs, develop and implement care plans, administer medications, and educate both patients and families on health management, while fostering strong relationships. You will work closely with physicians, therapists, and other healthcare professionals to ensure optimal patient outcomes. Responsibilities:
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status (OASIS)
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and adjust care plans as needed
- Educate patients and their families on managing their health conditions
- Collaborate with other healthcare professionals to coordinate patient care
- Maintain accurate and up-to-date medical records
Qualifications:
- Valid Registered Nurse (RN) or Practical Nursing (LPN) license in the state of Ohio
- Minimum of 1 years of nursing experience, preferably in home health
- Proficient in medical record systems and documentation
- Knowledge of aseptic technique and infection control practices
- Strong patient assessment skills
- Experience working with individuals with developmental disabilities is a plus
- Ability to work independently and prioritize tasks effectively
- Excellent communication and interpersonal skills
