Position Summary :
Chronic Care Management Coordinator conducts telephonic counseling to CCM patients and is responsible for documenting and communicating information to and from the providers for effective and coordinated care. This role involves working closely with patients, their families, and healthcare providers to develop and maintain personalized care plans, manage health conditions effectively, and prevent complications. The CCM Coordinator plays a critical role in fostering long-term health management and personalized relationships with the patients through regular communication, monitoring, education, and support, ensuring that patients navigate the complexities of managing chronic diseases while maintaining independence and wellness.
Responsibilities :
Ensures patient navigation is implemented by managing client caseloads, conducting intake assessment and reassessment, and supervising Patient Navigators.
Educating patients on health improvement strategies (e.g., blood sugar control, blood pressure management, reduction in hospital readmissions).
Ensure patient engagement in care plans by setting patient related goals
Team performance, including case management productivity, care coordination effectiveness, and patient adherence to care plans.
Successful identification and reduction of barriers to care (e.g., access to medications, transportation, social support).
Achieving departmental goals, driving performance improvements, and ensuring the program aligns with organizational objectives.
Skills and Abilities :
Strong team player but also could work independently
Strong understanding of medical terminology, light pharmacology, and the ability to read lab results.
In-depth knowledge of chronic disease management and patient-centered care models.
Excellent interpersonal and communication skills, including the ability to engage and educate patients and collaborate with interdisciplinary healthcare teams.
Proficiency in using electronic health records (EHR) systems and case management software.
Knowledge of insurance guidelines and coverage verification processes
Ability to navigate healthcare systems and community resources to support patients' needs.
Critical thinking skills, strong problem-solving and decision-making abilities.
Positive attitude and goal-oriented with a strong focus on achieving departmental objectives, driving performance improvements, and meeting key program milestones.
Qualifications :
Certified Medical Assistant hands on patient care is a must, minimum 2 years of experience
