Position Summary :

The Enrollment Coordinator is responsible for managing the enrollment process for patients, ensuring smooth transitions into healthcare programs, and handling administrative duties related to patient onboarding. This role supports patients, healthcare providers, and the internal team by ensuring all required documentation and compliance steps are completed. The Enrollment Coordinator ensures the accuracy, efficiency, and timeliness of the enrollment process for both new and ongoing patients.

Essential Duties and Responsibilities :

Enrollment Management :

  • Coordinate and oversee the entire enrollment process for patients into chronic care management (CCM), behavioral health integration (BHI), principal care management (PCM), and other healthcare programs.
  • Ensure that all required documentation (e.g., consent forms, eligibility information) is accurately completed and filed in accordance with program requirements and regulations.
  • Verify patient eligibility for enrollment in various programs, including health insurance verification and coordination with insurance providers.
  • Assist patients in completing necessary paperwork and provide guidance on program benefits and requirements.

Patient Communication :

  • Serve as the primary point of contact for patients throughout the enrollment process, addressing any questions or concerns regarding program eligibility and services.
  • Educate patients about the services offered, including care coordination, treatment plans, and any out-of-pocket costs or insurance coverage.
  • Follow up with patients to ensure timely completion of enrollment paperwork, and provide ongoing support and guidance.
  • Data Entry and Documentation :

  • Accurately input patient data into the Clinii system or other relevant databases.
  • Maintain organized records for all enrolled patients, ensuring that all documentation is complete, up-to-date, and compliant with HIPAA regulations.
  • Update patient files with any changes or updates to their enrollment status or related documentation.
  • Collaboration with Healthcare Team :

  • Work closely with healthcare providers (e.g., physicians, nurses, care coordinators) to ensure seamless integration of patients into care management programs.
  • Ensure that all necessary referrals and follow-up tasks are completed, facilitating smooth transitions from enrollment to active participation in care programs.
  • Quality Assurance and Compliance :

  • Ensure compliance with all applicable regulatory requirements, including HIPAA, CMS, and insurance guidelines, during the enrollment process.
  • Perform periodic audits to verify the accuracy and completeness of enrollment records and related documentation.
  • Scheduling and Coordination :

  • Schedule initial and follow-up appointments for enrolled patients with appropriate providers or care teams.
  • Coordinate any additional testing, assessments, or consultations required as part of the enrollment process.
  • Reporting and Tracking :

  • Track and report on enrollment statistics, including the number of successful enrollments, pending enrollments, and any issues encountered during the process.
  • Prepare regular reports for management, highlighting trends and opportunities for improvement in the enrollment process.
  • Qualifications :

    Education and Experience :

  • High school diploma or equivalent required; associate's degree or higher preferred.
  • Minimum of 2 years of experience in healthcare administration, patient services, or a related field.
  • Experience with enrollment or case management in healthcare programs (e.g., CCM, BHI, PCM) preferred.
  • Skills and Competencies :

  • Strong Communication Skills : Ability to clearly communicate complex information to patients and team members both verbally and in writing.
  • Attention to Detail : Ability to handle a high volume of paperwork and patient data accurately and efficiently.
  • Organizational Skills : Strong ability to prioritize tasks, manage multiple enrollments simultaneously, and ensure deadlines are met.
  • Customer Service Orientation : Strong interpersonal skills with the ability to build positive relationships with patients, families, and colleagues.
  • Familiarity with Healthcare Systems : Proficient in using Electronic Health Records (EHR) systems; familiarity with healthcare insurance processes is a plus.
  • Problem-Solving Abilities : Ability to identify issues or challenges in the enrollment process and proactively suggest improvements.
  • Knowledge of Regulations : Understanding of HIPAA, CMS, and insurance compliance as it relates to patient enrollment and data privacy.
  • Working Conditions :

  • This position typically requires working in an office environment with standard working hours.
  • Some flexibility in hours may be required, depending on patient needs or program deadlines.
  • The role may require occasional phone calls or virtual meetings with patients, providers or coordinators.
  • Physical Demands :

  • Ability to sit or stand for extended periods of time.
  • Ability to use a computer, keyboard, and other office equipment for extended periods.
  • Must be able to lift up to 15 pounds for handling paperwork or office supplies.
