Job Description
We are seeking a Registered Nurse (RN) Case Manager to oversee patient care coordination, focusing on social determinants of health, chronic disease management, and wellness-based care. The RN Case Manager will collaborate with medical providers, behavioral health professionals, and social workers to ensure patients receive holistic, high-quality care.
Key Responsibilities
- Conduct comprehensive patient assessments, identifying medical and social needs affecting health outcomes.
- Develop and implement individualized care plans, coordinating services for patients with complex health conditions.
- Provide education and counseling on disease management, nutrition, medication adherence, and wellness strategies.
- Coordinate with community resources to assist patients with transportation, housing, financial aid, and food assistance.
- Advocate for patients and ensure access to preventative care and specialty services.
- Serve as a liaison between patients, providers, and external organizations to improve care continuity.
- Monitor and track patient progress and follow-up needs to reduce hospitalizations and ER visits.
- Document case management interventions and patient progress in the electronic health record (EHR) system.
- Active RN license in New Jersey (or eligibility for licensure).
- Bilingual in English and Spanish (Fluency Required).
- Minimum 2 years of case management, patient navigation, or community health experience.
- Experience with chronic disease management, preventative care, and population health.
- Strong knowledge of social determinants of health and patient advocacy.
- Excellent communication, problem-solving, and patient education skills.
- Ability to work in a fast-paced, interdisciplinary setting.
- Experience in integrative medicine, wellness-based care, or aesthetics.
