Job Description: Social Clubhouse is a well-established provider of adult mental health partial care services in Springfield, NJ. We are looking for experienced Case Managers. The primary responsibility of the case manager is service coordination and member advocacy. The case manager will continuously assess and refer for services as needed on their caseload. The case manager will facilitate a daily Home Group in the morning and Wrap Up session at the end of the day consisting of members on their assigned caseload and collaborate with the Mental Health Specialists on curriculum for other therapeutic groups throughout the day. The Case Managers are the primary care coordinators for their caseload of members and work closely with other community providers and the site psychiatric provider to facilitate an effective plan for independent living.
· Group facilitation: creation of group curriculum in collaboration with other staff members.
· Individual sessions with each member on their caseload at least once a week.
· Creation of person-centered service plans in collaboration with each member on the assigned caseload, ensuring that goals are assessment driven and appropriate to the clinical assessment of each member.
· Responsible for checking in with members of their caseload regarding their medical, housing, and overall wellness needs and ensuring that these needs are met and woven into an effective service plan.
· Coordination of program services in collaboration with Mental Health Specialists
· Other responsibilities as assigned by program leadership.
Requirements:
· All Case Managers must complete training to provide the required group subjects: IMR, Prevocational and Co-occurring services, to fill in as needed for Mental Health Specialists.
· Possess a Bachelor’s Degree in a Human Services field, or an Associate’s Degree and two years’ experience in providing human services, or five years of human services experience, or possess a relevant professional credential such as Certified Psychiatric Rehabilitation Practitioner, Licensed Associate Counselor, Bachelor in Social Work, Certified Rehabilitation Counselor, Certified Alcohol and Drug Counselor, Bachelor in Rehabilitation, Bachelor of Science in nursing.
