Position Description
The Supported Community Living Specialist (SCLS) is responsible to the Team Leader and Director of Residential Services. This position is based in Malvern, IA.
Key Responsibilities:
- Teach, train, and assist individuals in developing skills for independent living, including health and medical needs, community participation, and daily activities.
- Orient new individuals in their home, assessing their skills, and providing input to progress reports and Person-Centered Plans.
- Supervise individual goals and complete data collection.
- Provide a supportive and therapeutic atmosphere.
- Assist with daily living skills, such as meal preparation, laundry, housekeeping, toileting, and bathing.
- Ensure clients' safety from environmental hazards.
Essential Functions:
- Provide quality care.
- Transport individuals.
- Train individuals in daily living skills, including meal preparation, laundry, cleaning, and personal hygiene.
- Maintain housekeeping standards.
- Work independently to ensure welfare and safety of persons served.
- Comply with state and federal regulations regarding HIPAA.
- Complete written and electronic documentation and record keeping.
Requirements:
- High school diploma or GED.
- Previous experience in teaching skills of cooking, cleaning, and personal hygiene preferred.
- Valid Iowa Chauffeur's license and acceptable driving record.
- Successful completion of pre-employment physical, TB Tine, and drug screenings.
- Certification in medication management.
- Ability to work with individuals with behavioral disorders and clean biological fluids/waste.
Physical Requirements:
- Ability to travel up to 70 miles.
- Ability to enter and exit resident homes and walk stairs with hands full.
- Ability to carry supplies up to 50 pounds and provide physical care to residents.
- Ability to perform routine household tasks and assist individuals with mobility issues.
