What are the responsibilities and job description for the Case Manager position at North Central Health Care?
The Opportunity: The Case Manager provides community-based case management for individuals diagnosed with mental health and/or substance use disorders. This role is part of our Comprehensive Community Services, Community Support Program, Children's Long-Term Support, or the Coordinated Services Team program.
Key Responsibilities:
- Determine ongoing eligibility for Community Treatment programs via the approved state functional screen
- Facilitate the assessment and service planning process, including identifying consumer needs, goals, objectives, strengths, barriers, interventions, and their priority in the treatment planning process
- Assess ongoing needs, including crisis situations, to determine treatment interventions and service provision
- Ensure the service plan and service delivery for each client is integrated, coordinated, and monitored
- Provide services in adherence to the pillars of Community Treatment program philosophy, which include being strength-based, recovery-oriented, person-centered, flexible, trauma-informed, and consistent with best practice
- Complete documentation in compliance with Community Treatment standards, agency policy, and applicable regulations
- Adhere to scheduled work hours with regular and predictable attendance
Requirements:
- Bachelor's or Master's degree in a human services field
- At least one year of case management experience
- Ability to work in a fast-paced environment
- Possess person-centered service in all interactions
- Valid driver's license and use of a personal vehicle
- Attention to detail and organized
- A competitive salary range of $26.50-$30.85, based on education and experience
- This job is full-time and in-person at our Merrill offices
