Position Title : Home Care Coordinator
Flexible Schedule : PT, Hourly 16hrs / week, On call weekends every 3rd-4th week.
Overview : The Home Care Coordinator is responsible for providing in-home care and support to clients in their home environment. This individual assists in the organization of family resources for the effective care of clients. In addition, this individual serves in a supervisory capacity to an assigned group of Homemaker Companions and provides training and coaching as necessary under the direction and in collaboration with the Clinical Care Manger, Registered Nurse, to Home Health Aides / CNAs. The performance of duties within this position is in accordance with CORE Values of The Village. This position reports to the Home Care Manager.
Essential Job Functions :
- Performs initial interviews and completes the necessary paperwork to sign prospective clients up for appropriate services and requests support from the Clinical Care Manager and Administrator as needed.
- Personally visits current associates in the field to ensure adequate work performance.
- Communicates observations or reported concerns regarding resident needs to the NFRV Home Care team.
- Provides ongoing, timely performance feedback and coaching to assigned direct care associates.
- Conducts annual and as needed performance evaluations for assigned associates.
- Serves as the liaison between the client, direct care associates and the Scheduling dept.
- Communicates with healthcare providers, families and responsible parties of residents as necessary to coordinate care.
- Maintains and ensures compliance with NFRV policies and procedures and AHCA regulatory standards.
- Personally visits clients per regulatory policy timing guidelines to ensure quality care.
- Maintains accurate, detailed reports and records in compliance with regulatory standards.
- Participates in Quality Assurance Committee Meetings which serve as a means for Performance Improvement process to ensure agency compliance with regulatory standards.
- Participates in the on-call rotation as assigned.
- Performs other duties as assigned.
