Position Title: Home Care Coordinator 

Salary Range: $29-32/hr

Flexible Schedule: PT, Hourly 16hrs/week, On call weekends every 3rd-4th week.

Overview:  The Home Care Coordinator is responsible for providing in-home care and support to clients in their home environment.  This individual assists in the organization of family resources for the effective care of clients. In addition, this individual serves in a supervisory capacity to an assigned group of Homemaker Companions and provides training and coaching as necessary under the direction and in collaboration with the Village Home Care Manager/Administrator, Registered Nurse, to Home Health Aides/CNAs. 

The performance of duties within this position is in accordance with CORE Values of The Village.  This position reports to the Home Care Manager and or the Director of Wellness & Community Life.

ESSENTIAL JOB FUNCTIONS:

Home Care:

• Performs initial interviews and completes the necessary paperwork to sign prospective clients up for appropriate services and requests support from the Clinical Care Manager and Administrator as needed.
• Personally visits current associates in the field to ensure adequate work performance.
• Communicates observations or reported concerns regarding resident needs to the NFRV Home Care team.
• Provides ongoing, timely performance feedback and coaching to assigned direct care associates.
• Conducts annual and as needed performance evaluations for assigned associates.
• Serves as the liaison between the client, direct care associates and the Scheduling dept.
• Communicates with healthcare providers, families and responsible parties of residents as necessary to coordinate care.
• Maintains and ensures compliance with NFRV policies and procedures and AHCA regulatory standards.
• Personally visits clients per regulatory policy timing guidelines to ensure quality care.
• Maintains accurate, detailed reports and records in compliance with regulatory standards.
• Participates in Quality Assurance Committee Meetings which serve as a means for Performance Improvement process to ensure agency compliance with regulatory standards.
• Participates in the on-call rotation as assigned.
• Performs other duties as assigned.

Vitality Clinic:

• Schedule and perform Vitality Assessments (wellness assessments) on Independent Living residents to include counseling, educating and referring to appropriate services if needed.
• Remain current regarding the healthcare and cognitive capability of the independent residents.
• Coordinate support services, i.e., home care, physical therapy, home health, etc.
• When assigned and working in Vitality, respond to PHB Alerts/emergency calls from residents throughout the campus and to provide first aid as needed, up to and including activating 911.
• Communicate with healthcare providers (physicians, therapists, ancillary providers, case managers) families, and responsible parties of residents.
• Counsel residents regarding maintaining a safe living environment in their independent apartments and follow through to ensure necessary changes are made by resident.
• Maintain current Independent Resident medical and contact information in MatrixCare as provided by residents according to the policy and procedure of the department and applicable regulations.
• Respond to all fire alarms and assist in the evacuation of residents when needed.
• Other duties as assigned

You Have: 

• Current and valid license in the State of Florida as an LPN required.
• Minimum of 2 years’ experience in a health care setting, with home care experience preferred.  
• Prior supervisory experience preferred.
• Ability to effectively manage the performance of others.
• Basic proficiency in Microsoft office products, specifically Word, Excel, PowerPoint and the ability to learn company proprietary software.  
• Ability to speak effectively in front of small to medium-sized groups.
• Clearance from human resources for pre-employment screening, i.e., TB, FDLE screening.
• CPR and First Aid Certification or eligible.
• Must have reliable transportation for travel to and between clients within Alachua County, Florida
