Job Summary
We are seeking a dedicated and compassionate HIV Care- Case Manager to join our Case Manager team. The ideal candidate will play a crucial role in coordinating client care, ensuring that individuals receive the support and resources they need throughout their healthcare journey. This position requires strong communication skills, a deep understanding of case management, and the ability to work collaboratively with other staff, healthcare providers, and community resource personnel across various settings.
Responsibilities
Assess client's needs and develop individualized care plans that address their medical, psychological, and social requirements.
Coordinate with multidisciplinary teams to facilitate effective treatment strategies for clients in various settings such as homes, clinics, and the criminal justice system.
Provide referrals to community services to include addiction counseling for clients struggling with substance use disorders and refer to in house smoking cessation so client can obtain surgery when applicable.
Ensure that MCM of HIV positive individuals includes treatment adherence, timely and coordinated access to medically appropriate levels of health and support services, and continuity of care.
Monitor client's progress and adjust care plans as necessary to ensure optimal outcomes.
Advocate for client's by connecting them with community resources and services that enhance their healthcare process.
Maintain accurate documentation of client interactions, care plans, and progress notes in compliance with regulatory standards.
Educate clients about available resources, HIV treatment options, and self-care strategies.
Participate in case conferences and collaborate with healthcare professionals to discuss complex cases and share insights.
Utilize the Department's CAREWare system to collect, track, and report timely client level and program-wide data and work with the Department annually to complete the Ryan White Services Report (RSR) that includes the Client Level Data XML (Extensible Markup Language) Report and Provider Services Report.
Other as requested by Executive Director or Supervisor.
This position must provide the overall case management duties and reporting requirements as outlined in subgrants.
Experience
A bachelor's degree in a health or human services field is preferred; or alternately, a candidate may have extensive experience defined as specialized case management training or an associate's degree and one to two (1 -2) years' experience in a human service-related field such as social work, psychology, or nursing.
Strong background in client care within settings such as non-profit organizations or specialized clinics.
Proven ability to manage multiple cases effectively while maintaining attention to detail.
Excellent interpersonal skills to build rapport with clients, families, healthcare teams, community resource providers, Executive Director and other staff is a must.
Knowledge of healthcare regulations and best practices in case management is beneficial.
Knowledge of Ryan White Part B federal grant guidelines is helpful.
