Description
Position Summary : The client services coordinator / Aide Liaison is responsible for ensuring contractual compliance, the management of client services, in addition to being the primary liaison between the nurses and the field staff.
Requirements
- Taking referrals from contracting agencies. Maximizing hours available while organizing initial weekly schedules, updating as necessary and minimizing travel and mileage expenses.
- Maintaining open communication and good relationships with contracting agencies.
- Quality control of employees' schedules on their EVV applications, making sure they are checking in and out of their clients' homes on time.
- Filing paperwork.
- Work closely with the scheduling supervisor to ensure services between clients, family members, and aides are coordinated in a timely manner.
- Maintaining personnel records and client files to ensure contractual compliance (hard copies and computerized).
- Coordinating with field supervisors to ensure that introductory visits are conducted in a timely fashion.
- Review client progress and referring concerns to the RN / Field supervisor and contracting agencies as deemed necessary.
- Attending agency meetings and off-site training as requested.
- Resolving routine client / employee conflicts and reporting concerns to the nursing supervisors.
- Cooperating with partnering Home Care Agencies in the coordination of services to mutual clients.
- Maintaining field staff informed policies and events via memos, staff meetings, phone calls, etc.
- Ability to be on-call after hours on a rotating schedule to assist field staff with emergency client issues and provide emergency coverage when necessary.
- Assist the Program Director with tasks as requested.
- Home visits to resolve immediate issues which may arise and refer concerns to nurses when necessary.
- Be on-Call and carry emergency phone line at least every other week when necessary.
- Additional tasks as deemed necessary for the efficient operation of the department.

Evaluation : The coordinator must continue to achieve a satisfactory performance evaluation annually. Performance evaluations are done by the Program Director with input from others in a supervisory and administrative capacity.
