Position Summary
CE Program
The North Shore Region’s Coordinated Entry (CE) Program is a standardized assessment, referral and housing placement process to ensure that households experiencing homelessness receive appropriate assistance with both immediate and long-term housing and service needs. Through this process, consumers with the most housing barriers are prioritized for the most cost and service intensive interventions. The North Shore Coordinated Entry process uses a “muti-point access” approach, while doing so through a standardized process from initial engagement to successful housing placement. Intake assessment and support is being provided at all the individual and family shelters in the region. Shelter and outreach staff will conduct CE screenings with consumers within their shelters. In addition, Action Inc., NSCAP and Emmaus Inc., act as regional intake centers for anyone seeking assistance and entry into the Balance of the State’s CE program but who is not currently staying in one of the participating shelters.
The Coordinated Entry Intake Case Manager is the first point of contact with households within the North Shore region who are experiencing homelessness or at imminent risk of becoming homeless.
CSPECH Program
The Community Support Program for People Experiencing Chronic Homelessness (CSPECH) funds tenant based services for individuals experiencing chronic homelessness. You would be working with these individuals with housing stability, addressing their health and other support services that the client may need or arise.
Requirements
Essential Duties and Responsibilities
· Conduct brief pre-screening assessments with households for homeless and/or housing assistance to identify immediate needs, connect consumers with appropriate community-based resources and assess their eligibility for and desire to participate in the Coordinated Entry program and the region wide waiting list for housing programs funded through this system.
· Meet with households in person or over the phone to discuss the Coordinated Entry system and conduct Coordinated Entry screening assessments with those interested in participating.
· Record Coordinated Entry assessment results on paper assessment forms or directly in the State’s VESTA HMIS.
· Ensure accurate and complete entry of Coordinated Entry assessment information at the time of enrollment into the CE program or the referral of an enrollment form to the CE Navigator at Emmaus who will enroll the person into VESTA CE HMIS.
· Be knowledgeable of data confidentiality and consumer confidentiality rights and be able to explain these rights to each consumer.
· Obtain a signed data confidentiality agreement from each consumer whose information is entered into the electronic Coordinated Entry Database for is being forwarded to the CE Navigator at Emmaus Inc. for submission.
· Report any capacity issues to the Coordinated Entry staff supervisor or designated agency supervisor.
· Refer consumers ineligible for homeless assistance services to other, more appropriate community resources.
· Receive and respond to calls from housing providers to assist in connecting with participants selected through the Coordinated Entry process to continue with an application for affordable housing.
· Assist participants selected through the CE System for an affordable housing opening with completing the application and documentation requirements to access the affordable housing program.
· Participate with CoC efforts to evaluate the effectiveness and efficiency of Coordinated Entry process within the agency.
· Maintain contact with the North Shore CoC Coordinated Entry Program administrative staff when requested to discuss concerns and issues around the Coordinated Entry process.
· Abide by the policies and procedures of the Coordinated Entry Program and attend mandatory trainings.
· Complete and maintain all necessary documentation in participant’s records and provide any additional reports or information on those served by the agency as requested by the Coordinated Entry supervisor or the lead agency Coordinated Entry Administrative Staff.
· Enrolling the clients in Ehana (database) and completing the required paperwork.
· Reach out clients that are in the CSPECH program twice a month to inquire about the goals that they have set forth.
· Perform any other tasks as deemed necessary by the Program Director of the Community Outreach Department.
· Participate in NSCAP trainings, staff retreats, or other meetings called by the Community Outreach Program Director.
· Other related duties as assigned by the Community Outreach Program Director.
Non-Essential Duties and Responsibilities
· Participate with other staff in the development and implementation of community wide strategies.
· Participate in the development and implementation of client education projects such as workshops.

· Sensitivity and ability to communicate effectively with people from various backgrounds and levels of disability
· Experience with low-income families and issues affecting them
· Strong interpersonal skills
· Own transportation required
· Understanding of the philosophy and goals of community action
Qualifications
· BA or BS in social services or BSW or a minimum of 5 years related work experience preferred
· Fluency in Spanish or Portuguese preferred
