QUALIFICATIONS:
- Current licensure to practice Physical Therapy in the state you are working
- Possess and maintain a valid Driver's License required
- Ability to work independently while adhering to established clinical protocols and guidelines
- Excellent oral and written communication skills
- Excellent organizational and time-management abilities to effectively manage caseloads/meet deadlines
- Possess the physical ability to perform job related duties, which may require sitting, lifting, standing, bending, stooping, stretching, walking, pushing, and pulling.
- A health screen or examination may be required prior to assignment and periodically thereafter, depending on the specific location policy, local or state regulations to verify physically capability to perform assigned duties with or without reasonable accommodations.
- Provide physical therapy services as prescribed by the physician that can be safely provided in the home.
- Conduct initial evaluations, and admission visit within 48-72 hours of referral for patients being admitted for physical therapy; in addition, develop a plan of care.
- Collaborate with a multidisciplinary team, including physicians, nurses, case managers, OT, to coordinate patient care and optimize treatment outcomes.
- Provide overall supervision of physical therapy assistants.
- Deliver evidence-based physical therapy interventions, including exercises, manual techniques, and modalities, to improve patients mobility, strength, and overall functional abilities.
- Maintain accurate and thorough documentation of patient progress in compliance with regulatory requirements.
- Educate patients and/or caregivers, families in the care and proper use of equipment and devices such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic and orthotic devices. In addition proper exercise, home safety and strategies for preventing injuries to promote long term wellness.
- Conduct comprehensive assessments of patients conditions and develop personalized treatment plans to address their specific needs.
