North Suffolk is looking for a Care Coordinator to join our Freedom Trail Clinic (FTC) Team! The Care Coordinator is responsible for the operations of clozapine clinic in North Harbor/Freedom Trail Clinics and for the care coordination of FTC outpatient clinic services. This is an on-site position within the North Harbor & Freedom Trail Clinic, 40 hours per week. Some off-site travel and/or work in other outpatient clinic sites may be required.
Language Differential: $5,000 for Spanish speaking candidates strongly preferred!
What You’ll Do:
- Maintains efficient operations of the clozapine clinic, and coordinates/delegates various functions to additional staff when needed.
- Assists with intake documentation and requirements.
- Educates and supports clients in accessing community resources (education, housing, employment, etc.) and aids with applications and referral documentation.
- Collaborate with a multidisciplinary team of social workers, counselors, nurse practitioners, psychiatrists, and administrators.
- Other duties as assigned.
What You'll Need:
- Education:
- High School Diploma or GED completion required.
- Associate’s degree preferred.
- Experience:
- Previous office experience, preferably in a healthcare setting, and computer skills required.
- Skills:
- Bilingual candidates strongly encouraged to apply! We offer a $5000 language differential pay! Please note language skills on your resume.
- Computer skills required to manage EHR medical record system, Word and Excel.
