What are the responsibilities and job description for the Hospice and Palliative Care Liaison LVN position at Northbay Healthcare?
The Hospice and Palliative Care Liaison LVN will receive all Hospice and Palliative Care referrals generated by the NBMC and VV Hospital staff and physicians. The Liaison is responsible for assessing, planning, implementing, monitoring and evaluating options and services for potential hospice and palliative care patients. The Hospice and Palliative Care Liaison LVN will act as a resource to Hospice Intake, The Palliative Care Team, NorthBay Case Management team, and outside referral sources.
- Education: Graduate of an accredited program for LVN. CHAA certified or certification within a year of hire.
- Licensure/Certification: Current LVN license to practice as a nurse in the state of California and per agency policy.
- Experience: Experience in hospice and palliative care is preferred plus one year in an acute care setting or hospice agency. Experience as a discharge planner, liaison nurse, or in home health/hospice intake preferred. Demonstrated experience in regulatory and standard of practice guidelines for /hospice/palliative care and reimbursement methodology.
- Skills: Knowledge of /hospice/palliative care regulations. Experience in working with third party payers and case managers. Good written, verbal and telephone skills.

- Hours of Work : Flexible hours to ensure business needs are met, occasional weekends/holidays as needed
- Compensation: $40.40 to $49..11 based on years of experience doing the duties of the job.
