Responsibilities:
- Shows successful ability in the care and management of residents with severe behaviors, safety and monitoring of residents.
- Has knowledge of, and implements, the instructions given on the Behavior Care Plan of the residents.
- Records and documents the data collection and analysis of residents behavioral episodes in the Behavior Log Book on a regular basis.
- Records the movement of residents exiting and entering the building in the CSS Log Book on a regular basis.
- Address coverage concerns on all posts and shifts.
- Assists and monitors residents in therapeutic groups and other activities.
- Crisis interventions with challenging behaviors/residents when necessary.
- Monitor and enforces neighborhood (unit) rules and policies.
- Accountability of monitors the resident “Pass System”
- Assists and monitors residents on community outings, trips, and medical appointments.
- Documents the CSS patrols conducted during their respective shifts.
- Has a working knowledge of approaches for difficult residents off of unit, and the rules policies of the facility outside or the designated unit.
- Ensures a clean and safe environment for residents and co-workers.
- Follows attendance policy and guidelines as set forth by the facility.
- Attends CSS meetings.
Requirements:
- Perform 1:1 (CVO) coverage when necessary, for the safety of the residents following the residents Care Plan
- Performs Increased Supervision (IS); which includes visual confirmation of neighbors every 15 minutes.
- Provides hallway coverage for safety and consistent enforcement of facility policies as needed.
- Maintains organization of the unit to avoid therapeutic milieu from surfacing from the residents.
- Implement, record and monitor Credit and Level system for NBI residents.
- Data collection and analysis when required for individual Care Plan Goals
- Assist residents with individual and group goals
- Acts in role of staff advocate for “Personal Group when assigned.
- Perform any related functions directed by CSS –Department Head
- Responds to crisis situations and “ Code Rainbows “
- Assign shift CSS to unit/tasks
- Monitor CSS performance during “Code Rainbow” or therapeutic intervention
- Document performance on CSS audit toll if indicate
- Cover the assignment when necessary
- Address troubleshooting concerns as they arise
- Assist in scheduling adjustment and attendance
- Perform any related functions as directed by the CSS Head of Department or his/her designee
- Collaborates with Nursing Supervisor and/or Unit Charge Nurse as indicated
- Monitor shift to shift communication
- Inservices, disciplinary reports
- Training of new staff
