What are the responsibilities and job description for the Community Integration Case Manager position at Northeast Counseling Services?
The Prison Community Integration Case Manager engages individuals as a handoff from Northeast Counseling Services' Prison Reintegration Case Manager after their release from jail. These individuals may be diagnosed with a mental health or drug/alcohol concern. This case manager provides support, resources, and empowerment, ensuring individuals receive and follow up with the necessary community services to decrease the chance of reincarceration. The case manager will follow a strengths-based approach, working with the individuals in identifying social, vocation, occupation, and self-maintenance needs, including family involvement. The primary goal is to empower the individual to identify their needs and choose the resources to fulfill them. This workforce member has access to the complete chart for cases requiring his/her worker's involvement.
QUALIFICATIONS/EDUCATION: The position requires a Bachelor's Degree in social work, psychology, or a related field from an accredited college or university, as well as, knowledge of the mental health, drug and alcohol and social service field. The individual should possess good judgment, positive communication skills, and the ability to organize their time to meet consumer needs in a flexible, non-traditional time frame. They must be self-motivated and have the ability to be creative and aggressive in seeking services for consumers.
SALARY: Up to $37,000
