SUMMARY OF RESPONSIBILITIES:
This person will function as a member of the Assertive Community Treatment (ACT) Team. Primary areas of responsibility are nursing assessments to include monitoring of health care needs, collaboration and monitoring of the individual served, social, community, and vocational/educational functioning. This will include obtaining and documenting all necessary information as mandated in the agency Case Recording Guidelines. Must work within a team framework to determine the most appropriate treatment regimen in meeting the identified needs of this population. Will be an active member of the after hours on-call rotation for the ACT Program, providing emergent assessments as necessary.

Rate of Pay: $54,870 - $75,774

FLSA: Full Time/Exempt

Report To: ACT Clinical Supervisor

QUALIFICATIONS:
Education: Registered Nurse Degree from a recognized or accredited institution with course working in mental health. Possess State registration as a Registered Nurse.
Experience: One or more years of relevant mental health experience. Lived experience with mental illness, intellectual developmental disabilities, and/or substance use disorders valued.
Other: Functional computer literacy is required. Good communication skills (both verbal and written) are required. Must have Michigan driver's license and a satisfactory driving record as determined by the agency.

ESSENTIAL JOB FUNCTIONS:
1. Demonstrates understanding of assessment and diagnosis for persons requesting Mental Health Services in accordance to the latest edition of the DSM and ICD.
2. Demonstrates effective case recording in accordance with Person Centered Planning, and Agency "Case Recording Guidelines."
3. Provide clinical assessments and treatment in the least restrictive environment (agency, church, individual's home, etc.)
4. Provides education, crisis intervention and individual and/or family treatment as necessary.
5. Collaborates with other staff in diagnosing problems and evaluating clients, as needed.
6. Consults with various agencies, individuals and organizations.
7. Will be an active member of the after hours on-call rotation for the ACT Program, providing emergent assessments as necessary.
8. Staff will be expected to participate in trainings aimed at the provision of services for individual's with cooccurring disorders Integrated Dual Disorder Treatment (IDDT) Fidelity.
9. Participates as an active member of the Children's Diagnostic and Treatment Team.
10. Demonstrates knowledge of universal precautions, standards of practice including but not limited to confidentiality and recipient rights, and emergency protocol.
11. Work flexible hours to meet the needs of the program.
12. Complete associated ACT training annually to be in compliance with model fidelity.

NON-ESSENTIAL JOB FUNCTIONS:
1. Performs other related duties as required.
2. Maybe requested to be involved in the Board's continuous quality improvement program.
3. Maybe required to provide own transportation while performing agency-related duties.
4. Will be required to participate in the provision of 24-hour emergency services through the ACT and agency's Emergency Services.
