SUMMARY OF RESPONSIBILITIES:
The Qualified Mental Health Professional in this position will participate in providing active treatment as well as other mental health services to persons residing in nursing homes, AFC homes, and independent living. This individual will participate in assessing the biopsychosocial needs of individuals with severe and persistent mental illness and/or intellectual/ developmental disability. They will be responsible for engaging the consumer, their guardian, family members, or significant others in the person-centered planning process and monitoring progress on goals and objectives to promote and support recovery. The individual will provide direct treatment services and/or link consumers to necessary ancillary community services. The individual will provide Nursing Home Mental Health Monitoring services as part of a coordinated treatment team to ensure that care plan objectives related to mental health needs are carried out within the nursing home. This position will also entail the provision of Targeted Case management services or Supports Coordination services to individuals residing independently or in dependent care placements in the community. This individual will be responsible for providing consultation, educational, and training services to nursing home staff and individuals/groups within the community designed to improve mental health awareness, knowledge, and treatment of mental health issues of the older adult population. The individual may be involved in assisting with the OBRA Level I and Level II process as necessary.

Rate of Pay:

  • BA: $21.40 - $29.50
  • RN: $26.38 - $36.43

FLSA: Full Time

  • BA: Non-Exempt
  • RN: Exempt

Reports To: Older Adult Services Supervisor

QUALIFICATIONS:
Education: Bachelor’s Degree in Social Work (or other related field) or Registered Nurse from a recognized or accredited institution.
Experience: One or more years of relevant work experience working with the older adult population. Good communication skills (verbal and written) are required. Must possess current Michigan license as a bachelor’s Level Social Worker or Registered Nurse. Functional computer literacy is required within 6 months of the date of hire. Lived experience with mental illness, intellectual and developmental disabilities, and/or substance use disorders valued.

ESSENTIAL JOB FUNCTIONS:

  • Demonstrates effective case recording in accordance with Person-Centered Planning and Case Recording Guidelines established by Agency Policy and Procedures.
  • Responsible for the application of and compliance with Omnibus Budget Reconciliation Act regulations and procedures. A working knowledge of applicable regulations is required.
  • Demonstrates accurate assessment and diagnosis for persons requesting mental health services in accordance with the most current editions of the DSM and ICD.
  • Responsible for provision or arrangement for necessary therapeutic and support services for nursing facility residents and older individuals residing in the community with severe or persistent mental health need or intellectual/developmental disabilities.
  • Responsible for being an active member of the nursing home’s care planning team for individuals in need of mental health services.
  • Responsible for providing consultation and training services to nursing facility staff and individuals and groups within the community regarding mental health and aging.
  • Will demonstrate functional computer literacy in all agency documentation.
  • Will be required to maintain the highest degree of licensure specific to their position.
  • Demonstrates knowledge of universal precautions, standards of practice, including but not limited to confidentiality and recipient rights and emergency protocol.
  • Staff will be expected to participate in training aimed at the provision of services for consumers with co-occurring disorders.
  • Demonstrate effective collaboration with agencies and individuals associated with OBRA Services (hospitals, nursing homes, AFC Homes, physicians, families, counselors, and consumers).
  • Demonstrate effective collaboration with individuals, groups, and agencies within the community to allow for effective mental health case management/supports coordination services.
  • Demonstrate knowledge and understanding of the aging process and factors that may impact the mental health needs of the older consumer.
  • Demonstrate the ability to work effectively in a team environment.
  • Will be required to participate in the Emergency Services Unit’s rotating on-call system; will be required to complete 24 hours per fiscal year of annual training specific to children (24 CE hours according to Policy 5135).

NON-ESSENTIAL JOB FUNCTIONS:

  • Performs other related duties as required.
  • May be requested to be involved in the Board’s continuous quality improvement program.
  • May be required to transport consumers and/or provide own transportation while performing agency-related duties.
  • Prepares various reports related to duties as required.

Ability to walk, bend, stand, sit, lift up to 25 lbs., with or without assistance, stretch/reach, hear, see, hand/finger dexterity, and drive.
