DescriptionSummary of Duties : Provide a variety of office and field activities to manage and monitor transitional housing and rapid rehousing to clientsas an alternative to unsheltered homelessness. The YHDP program is designed to engage with young adults ages 18-24who have experienced homelessness to provide temporary transitional housing with support services while locatingpermanent housing. Work involves aligning and providing housing search and supportive services to promoteparticipant self sufficiency, integration into the community and maintaining permanent housing. Position is alsoresponsible for educating potential landlords, the public, and community partners on programs offered by the agencyand the impact they have while building support and political will for ending homelessness. Frequent travel throughoutthe 11-county service area and to other locations in the state for meetings, workshops, trainings and conferences asrequired. - Assess housing barriers of individuals and families of youth and young adults between the ages 18-24 experiences homelessness to determine housing and service needs.
- Assist participants in locating and securing safe and affordable housing.
- Develop a housing, financial and self-sufficiency case management plan with participants.
- Complete thorough housing inspections of rental units.
- Provide advocacy and mediation with landlords to develop a workable plan to obtain or maintain participant's housing.
- Complete all housing documentation and process payment requests for security deposit and ongoing rent as necessary.
- Assist participants in development of a strength-based / solution-focused individualized goal and action plan that promotes permanent housing and self-sufficiency.
- Identify participant strengths and barriers to stability and assist participants to reducing barriers and linking to resources and services.
- Effectively utilize multiple means of technology, including computers, cell phone, Microsoft Office products and others as required to complete daily job-related functions.
- Maintain knowledge on program guidelines and follow them appropriately to keep in compliance with the grants.
- Enter information into the Homeless Management Information System database for each family / individual served in a timely manner.
- Attend appropriate local action committees, planning groups, departmental meetings and inter-agency service teams as required.
- Must have the ability to work in a team setting and also independently to accomplish all required tasks.
- Provide information and referral assistance regarding available support from appropriate community programs.
- Conduct community presentations as needed to promote all assigned housing programs.
- Meet with participants regularly to provide support services for housing stability as outlined in their housing plan.

An education degree with appropriate course work and / or experience will be considered.Certificates, Licenses, Registrations : Persons in this position must have reliable transportation and a valid Michigan driver license.Language Skills : Ability to read, analyze and interpret written materials. Write reports, business correspondence and manuals. Presentinformation and respond to questions.Mathematical Skills : Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions anddecimals.Reasoning Ability : Ability to define problems, collect data, establish facts and draw valid conclusions.
