Resident Health Navigator

Northeast Pennsylvania Area Health Education Center (NEPA AHEC) is the Operating Site for the National Health Corps (NHC) Community Health Fellowship (CHF) Program under the umbrella of AmeriCorps. The NHC CHF is a grass roots community health service program that trains local residents in Greater Philadelphia, Northeast Pennsylvania, Delaware and Centra California as community health workers, digital navigators, and / or medical interpreters who are committed to addressing the unmet health needs of underserved populations and developing grass roots community health leaders.NHC CHF members participate in the PA AHEC state approved Community Health Worker training and complete 1700 hours of service during a 46-week service term with one of our host sites. Members receive a $15 / hour living allowance during, medical, dental and vision coverage if needed, assistance with childcare if eligible, and a $7395 Educational Award at the completion of the service year that can be paid to existing student loans or used for future educational opportunities.St. Joseph's CenterRooted in the core values of care, concern, compassion and commitment, Saint Joseph's humbly serves people who are diagnosed with intellectual disability and those who seek pregnancy support, adoption assistance, outpatient therapy or medical day care services.The NHC CHF member in the Resident Health Navigator Position with St. Joseph's Center would be responsible for advocating for quality medical care for 88 individuals who reside at the Scranton Intermediate Care Facility. The individual in this position will advocate for meaningful interactions and opportunities within the community. The Resident Health Navigator is responsible for completing the medical profile that accompanies each individual when they are sent out to the hospital. The member will accompany individuals to their medical appointment when a nurse is not required. They will ensure that they can provide an accurate representation of the individual's issues or concerns. They will also relay pertinent information to the nursing and medical records department following the appointment.Qualifications for these positions : -Able to build strong relationships-Community engagement / outreach skills-Familiar with health equity issues-Strong attention to detail and data entry experience-Organizational and time management skills-Ability to work independently-Exemplary listening and communication skills-Collaboration skills-Self-motivated, proactive, and creative problem solver
