$500 Sign-on Bonus Available

Under the supervision of the Director of Outpatient and Partial Program, the FFS Clinician  is responsible for individual, family, and group therapy provided to the consumers on his/her caseload, with therapy sessions scheduled once per week and family therapy scheduled as recommended / necessary.  Clinicians are responsible for scheduling and maintaining his/her own appointments and communication with families / clients about appointments.  Clinicians will maintain consistent weekly schedule with clients / families and will notify supervisor in advance of schedule or any schedule changes. Clinicians are responsible for all documentation including progress notes, treatment plans (OP/PHP), communication letters, discharge summaries, aftercare plans, discharge notes, and discharge letters. Clinicians will maintain consistent communication with supervisors, psychologists, and consulting and medicating psychiatrists, and other treatment team members to facilitate care coordination.

SPECIFIC DUTIES:

Communication

  • Maintain therapy appointments with consumers and their families, including following up on cancellations and no-shows.
  • Maintain correspondence with families who do not show for sessions or are unable to be reached via telephone through letters.
  • Obtain signatures from consumers and their guardians on all paperwork requiring them on specified dates as the paperwork is prepared.
  • Maintain consistent correspondence with supervisor regarding continuing treatment.
  • Notify Supervisor of any emergency situations within 30 minutes. Follow agency emergency protocol and complete incident report within 24 hours.
  • Attend supervision as scheduled.
  • Consult with psychiatrist and psychologist as directed and provide clinical updates as needed / requested.
  • Clinicians are expected to check mail boxes, email, and voicemail on a daily basis.
  • Communicate with OP/PHP case manager when additional support to meet paperwork requirements is necessary at the discretion of the supervisor.

Direct Care 

  • Provide child-centered, family-focused individual, group, and family therapy to assigned and scheduled caseload.
  • Encourage family involvement in treatment and document all attempts to include families when necessary.
  • Provide Group therapy for PHP program as directed.
  • Provide crisis intervention and action in response to acute client behavior, including oversight of CRC transportation and accompaniment if necessary for OP and PHP consumers as directed.
  • Provide risk assessments when necessary for OP and PHP consumers as directed.

Documentation

  • All paperwork is to be completed to the specifications described below and submitted in an organized fashion by 12 noon on Wednesday.
  • Clinicians will complete GDAP notes and encounter forms for each therapy session completed.
  • Complete progress note for any therapy appointments that do not occur as scheduled on a weekly basis.
  • When necessary complete OP treatment plans every 90 days or 10 sessions reflecting current goals and progress
  • Complete a discharge summary, discharge note, aftercare plan, and follow up letter for each consumer upon discharge from the program within one week of discharge.
  • Document all telephone contacts with consumers, their families and outside agencies via clinical progress note.
  • Follow up with families who can not be reached via telephone through communication letters.
  • Track and update consent forms annually or as needed.

Planning 

  • Schedule to meet with consumer guardians to complete all treatment plans.
  • Set up and maintain a weekly therapy schedule for his/her caseload as assigned by the Director of Outpatient & Partial.
  • Clinicians are responsible for keeping track of paperwork; including dates on treatment plans, progress note numbers, and treatment plan goals.
  • Maintain charts via clinical documentation for all assigned clients.
  • Clinicians are responsible for keeping personal supplies of all necessary clinical documentation including, consent forms, HIPPA forms, welcome letters, treatment plans, progress notes, discharge summaries and aftercare plans. Asst. Director / Directors have documents to place on disk or to email.

Additional Duties

  • Attend all agencies specific trainings and additional professional trainings as assigned.
  • Complete chart reviews as assigned
  • Complete discharge paperwork and aftercare plans as assigned.
  • Provide administrative and case management duties as assigned.
  • Maintain professional and agency ethics, follow all agency polices and procedures, and work in accordance with professional bodies.
  • Assist PHP staff as needed.

CORE COMPETENCIES:

  • Ability to communicate effectively and interact professionally with all staff and clients.
  • Ability to organize and communicate with a caseload independently.
  • Ability to deliver effective therapeutic services within the context of individual and family therapy.
  • Ability to complete paperwork up to standard and on time.

Education 

  • Minimum Master’s Degree from an accredited university in a clinical mental health discipline, such as psychology, counseling, clinical social work, and marriage and family therapy.  The degree must include at least twelve (12) credit hours in psychology.

 Experience

  • At least two (2) years’ experience working with children in a mental health setting.

 Special Skills: 

  • Good verbal and written communication skills. 
  • Ability to interact with people in varied settings.

 PHYSICAL CAPABILITIES AND OTHER REQUIREMENTS:

  • Ability to walk up and down steps.
  • Ability to perform De-escalation Techniques in a safe and appropriate manner.
  • Ability to lift 20 lbs.
