Social Services Assistant – Full-Time
Northern Dauphin Nursing & Rehab | Millersburg, PA
Northern Dauphin Nursing & Rehab is seeking a compassionate and dedicated Social Services Assistant to join our team. In this role, you will support residents and their families by advocating for their social and emotional well-being while assisting in the overall coordination of our Social Services Department.
Responsibilities:
- Assist in developing individualized care plans that address residents’ psychosocial and emotional needs.
- Support the admission and discharge process, ensuring a smooth transition for residents and their families.
- Provide emotional support through individual and group interactions during times of adjustment, crisis, or need.
- Maintain detailed documentation of residents’ behavioral and psychosocial status.
- Assist in coordinating volunteer and community engagement programs that enhance residents’ quality of life.
- Educate and orient residents and families to facility services, policies, and available resources.
Qualifications:
- Bachelor’s Degree in Social Work or a related field.
- Experience in a long-term care or healthcare setting preferred.
- Strong understanding of federal and state regulations related to social services in skilled nursing facilities.
- Excellent communication, organizational, and interpersonal skills.
